* Original passport valid for at least 6 months from the date of entry into Philippines

 * A minimum of three blank pages on the passport

 * All old passports, if any 

* Two passport size photos
* The passport size photo should be 35 mm wide by 45 mm high and taken within the last three months

* The photograph must reflect true skin tone and must be evenly bright

* The facial expression must be neutral with the mouth closed
* The photo must be taken against a white background with the hair and shoulders visible 

* The wide should be kept wide open but if the applicant typically wears glasses, they must do so in the photo as well

* Headgear is acceptable for religious purposes only and should be in a dark colour 

* The photo should not be distorted or flipped 

*Original, signed letter from inviting party indicating complete address and telephone number. The letter must also be notarized in the Philippines.

* Employed applicants must produce bank statements proving sufficient funds & employment

* If self-employed then their company’s bank statement is required

* If being sponsored, the traveller must provide the sponsor’s bank statement

* All applicants must provide 6 months bank statement signed and attested by a bank official

* All bank statements must be dated within the last week of application & be verifiable

*If minor, under 18 years of age traveling alone or to be accompanied by a person other than a parent or not joining a parent in the Philippines: Affidavit of Support and Consent to Travel. The affidavit must be executed by either or both parents indicating traveling companion of minor and address in the Philippines, duly notarized by a local Notary Public, attested by the MEA and consular officer of the Philippine Embassy. This affidavit must be presented before immigration officials at the port of entry in the Philippines upon the minor’s arrival in the Philippines.

*Cover letter. For self employed covering letter on letterhead.

*Income tax returns / Form 16 for the last year.

* Proof of address if the address on the passport differs from the current place of residence

 * Two-way air tickets

 * Credit card copy and last 3 month’s statement or foreign exchange receipt;

 * Visa application form 

 * Hotel confirmation/ proof of accommodation
